A PARTICIPATORY MODEL FOR EVALUATING SOCIAL PROGRAMS
Written by a team from UCLA’s School of Public Policy and Social Research, the report describes a unique approach to evaluation that is gaining momentum in the nonprofit sector. The team’s leader, professor and professional evaluator Dr. Yeheskel “Zeke” Hasenfeld, calls the approach “participatory,” because it emphasizes client participation in every aspect of the evaluation process.

BNPCA - ICDM Discussion Paper
Integrated Chronic Disease Management Discussion Paper prepared for BNPCA Strategic Partnership Group meeting discussion October 2008. The paper is based on the current models of chronic disease care and the series of consultations undertaken with BNPCA agenices involved in the care of people with chronic disease. From this discussion an ICDM Strategic Statement of Intent was drafted and agreed upon, and now forms the basis of the BNPCA approach to Integrated Chronic Disease Management work in the catchment.

Better Health Self Management - Evaluation Results July 2003
Better Health Self Management Programs make a difference!! Preliminary results are in from the “Take the Pressure Down” Better Health Self Management programs (BHSMP)! As part of the “Take the Pressure Down” external evaluation, telephone surveys were conducted 6 months post program completion with participants from the first 8 programs. 47 participants consented to be interviewed. Majority of participants reported positive effects on health and improved ability to manage blood pressure Congratulations to the local leaders of the programs with 98% of consumers very satisfied with the way the leaders interacted with them. Further evaluation of the programs is continuing and final results will be reported in December 2003. Care in your Community - Trials Report
As part of the implementation work, CiYC was trialled across three sites with evaluation of the implementation methodology being done. This evaluation was completed by independent consultants and is available below. The consultants have submitted their final report following close consultation with CiYC governance structures. The trials of CiYC were successful; there was strong stakeholder engagement and consensus on a range of areas. Combined Impact of Health Behaviours
There is overwhelming evidence that behavioural factors influence health, but their combined impact on the general population is less well documented. This study aimed to quantify the potential combined impact of four health behaviours on mortality in men and women living in the general community. This study examined the prospective relationship between lifestyle and mortality in a prospective population study of 20,244 men and women aged 45–79 y with no known cardiovascular disease or cancer living in the UK.

Describing and analysing primary health care system support for chronic illness care in Indigenous communities in Australia's Northern Territory - use of the Chronic Care Model
Using concrete examples, this study translates the concept of the Chronic Care Model (and associated systems view) into practical application in Australian Indigenous primary care settings. This approach proved to be useful in understanding the quality of primary care systems for prevention and management of chronic illness. Further refinement of the systems should focus on both increasing human and financial resources and improving mangement practice.

The Management Committee of the Electronic Service Coordination System (E-SCS) has commissioned Ocean Informatics to perform this evaluation of the current state of the application and its use in the Eastern region. E-SCS has been implemented in an increasing number of sites over the past few years but it has only been in the last 12 months that a concerted effort has been made to train staff and encourage wider use. This recent effort has resulted in a very encouraging increase in the e-Referral traffic amongst a wide range of users. This report is designed to understand how the current system is being used and accepted and what is needed to sustain and increase its usage. There is also a need to understand how the system fits with other initiatives in the electronic Referral domain in Victoria and further afield. The evaluation process undertaken used two complementary approaches. It commenced with an online survey to get feedback about the broad on-the-ground experience of E-SCS. Registered users and managers were emailed invitations to participate in the survey; results were collated and analysed. Results from the survey were shared, and the issues raised in the survey were discussed, with focus groups and stakeholders to obtain more detail and practical insight. Conclusions and recommendations comprise the final part of the report, based on collation and studying the combined feedback from the online survey, focus groups and stakeholders.

GP Engagement Strategy Sub-Report

The gender and diversity lens is a quality improvement resource which has been developed with input from a range of funded organisations, departmental staff and consumers. It is a priority action for the second stage of the Victorian Women’s Health and Wellbeing Strategy and consistent with the State Government social policy framework, A Fairer Victoria. The lens will assist Department of Human Services programs and funded specialist and mainstream services take into account the interaction between gender, diversity and disadvantage during the planning cycle and service delivery. HACC Assessment Framework
The HACC Assessment Framework sets out progran policy for Assessment as a HACC funded activity. It describes in detail the requirements for the delivery of a Living at Home Assessment which includes a home-based holistic assessment of need and service-specific assessments as key components. In most instances theses Assessments will occur wherever the client is currently living in the community. The Framework also describes related processes such as Client Care Coordination and Supported Acess. Both of these processes are critical adjuncts to assessment for specific client groups. The policy described in the Framework builds on existing assemssment expertise and good practice within the HACC Program.. To this end, the Framework will be an important reference point for promoting more consistent assessment processes and practices across Victoria.

HPC Resources Guide on Evaluation Methods
People involved in health promotion work are becoming increasingly interested in evaluating their programs and interventions. Evaluation can provide information to aid in the planning of a program (needs assessment), information on how the program is operating (process evaluation) and whether it has had its intended effect (outcome evaluation). While the primary goal of evaluation is to provide “useful feedback” about the program/initiative to stakeholders (i.e., clients, program organizers, funders, staff, etc.), it is also an important source of future evidence to help develop other programs or make adaptations to your own program.

Hypertension Project - Consumer Consultation Report
Late last year a number of interviews and focus groups were run by the project. The purpose was to gain a better understanding of people's perception of high blood pressure, what issues were concerning them most about having high blood pressure and what type of program they felt may be able to help them deal with these issues. The findings have now been collated and the final report completed. The information provides direction for the ongoing planning of the program. The report contains a wealth of valuable information that could be useful for anyone planning programs for people with chronic health problems or community consultation.

Improving Access to Mental Health Services Forum
The Forum included speakers from a variety of organizations including; The Banyule Nillumbik Primary Care Alliance, North East Primary Mental Health Team, Banyule Community Health, Austin Health, Nexus and a consumer. The information presented included an overview of the prevalence of mental health disorders in the Banyule-Nillumbik catchment and some of the issues that affect access to services by these clients. This was followed by a description of the various pathways that might exist for a client within the Acute/Sub-Acute and Community sectors with brief information on eligibility and the referral processes involved. The last part of the session provided strategies that agencies can implement to improve service access for clients with mental health issues. The Speaker presentations are attached below in a zip file. This session was designed as an introduction to the various elements of the Mental Health system. Due to the complex nature of the topic, the Adult system was the main focus of the forum, however some printed material and resources was available on Infant/Child, Youth, Aged and Specialist Mental Health services as well as Community supports. The forum allowed workers to ask questions about various aspects of the Mental Health system and this contributed to a very positive outcome for the forum. Catering was provided by the Asylum Seeker Resource Centre and over food, much networking and further discussion about promoting Mental Health pathways and services was undertaken. Thank you to all the speakers and those who helped out on the day. Also thank you to all those who attended on the day or requested information to take back and share with other staff at their place of work.

Long term conditions compendium of information
This documentfrom the UK and launched in January 2008 updates the first compendium of information on LTCs, published in May 2004. It will further inform all those who are involved in both commissioning and providing care and support services for people with Long Term Conditions (LTCs). It focuses on the outcomes that people with LTCs said that they wanted from services and describes how more effective management of LTCs in a number of local communities is delivering high-quality and personalised care. Low-income Groups and Behaviour Change Interventions
A recent article published by the King’s Fund in London, reviewed the evidence base for the effectiveness of health behaviour interventions that target low-income groups, with the aim of reducing smoking, unhealthy eating, or increasing physical activity. One of the premises of the review was that “reducing health inequalities depends on developing interventions to increase health behaviours that are differentially effective in favour of those from disadvantaged backgrounds or target socially disadvantaged groups”. After an extensive screening process the review was based on 17 papers that fulfilled the criteria set by the researchers. The review highlighted two suggestive findings: • More focused interventions involving a small set of techniques may be more effective than interventions combining a large number of different techniques; which highlights the need to monitor the fidelity of intervention delivery. • The most common techniques (providing information and facilitating goal-setting) may be helpful for low-income groups. The two sets of techniques may be working additively, in that providing information about the benefits of changing behaviour may increase peopoe’s motivation to change, while helping people to form specific, realistic goals help people to translate motivation into action. The paper concludes that reducing health inequalities needs increased investment in research into behavioural interventions for disadvantaged groups. This brief contact list was put together to provide a listing of Mental Health agencies within Banyule and Nillumbik. Partnership Self-Assessment Report - April 2009
The information in this report is based on data that the Strategic Partnership Group members of the Banyule Nillumbik Primary Care Alliance provided when they completed the Partnership Self-Assessment Tool1 questionnaire in 2009 and is compared with the 2007 results. Overall, the BNPCA has improved its self-assessed rating in most items on this Partnership Self-Assessment Tool. Most importantly all except one item that was specifically targeted for improvement was enhanced. Current planning processes underway will address some of the items highlighted for improvement in this report and others will be clarified to determine their relevance for this Alliance.

Planning for a Healthier North
This is a Report on "the Planning for a Healthier North Leadership Forums 2006" which aimed to increase awareness of, and explore options for the establishment of a regional, integrated health planning model. To achieve this a series of four forums were conducted, with the final forum being held on November 16, 2006. The agencies that attended the forums were from the seven local government areas of Moreland, Hume, Yarra, Darebin, Whittlesea, Banyule and Nillumbik and included representation from: · 7 Local Governments · 10 Community Health Services · 1 Royal District Nursing Service · 1 Victorian Aboriginal Health Service · 3 Divisions of General Practice · 3 Primary Care Partnerships · 7 Metropolitan Health Services The majority of organisational representatives were CEOs with the exception of Local Government where Directors of Community Services were asked to attend. Each forum had a specific focus, and common themes were explored across all. The forums were designed to facilitate group discussion and explore some of the realities of what regional integrated planning might mean. The common theme across all forums was the benefits of co-operation and working together. The importance of trust and sound partnerships were emphasised. Real examples were presented and discussed and the challenges and benefits of integrated planning were highlighted. Forum 1 focussed on principles for priority setting, what is important to agencies in the North and what are some of the underlying values. These principles will drive the regional integrated planning model. Forum 2 considered processes for how decisions are made and how agencies can work together. This was done through a group exercise of allocating a hypothetical $150 million across the region. Forum 2 highlighted processes that will be important to incorporate into the workings of the model. Forum 3 examined planning cycles — how should the planning that is currently occurring relate to a regional integrated planning model? Which planning work needs to be linked in with a regional model? Forum 4 looked at the issue of planning boundaries. What are the current boundaries, what works well and what boundaries make sense for a regional integrated planning model? There was also discussion of other aspects of the model that require additional consideration, including a vision for the Northern community and defining the scope of the model. Practice Change: Learnings from the Integrated Chronic Disease programs
This is the report of a research project that was a collaboration between three Melbourne-based Community Health Services: Knox, MonashLink and Darebin Community Health Services with the Department of Health Science at Monash University. Staff from the Department of Health Science undertook the research which was funded by a grant from the Helen Macpherson Smith Trust. The study aimed to identify changes to management and care practices within and across organisations, and how individual health workers can further develop the necessary skills that support improvements in care for people with chronic medical conditions within Victorian Community Health Services. The qualitative research methods used included a review of relevant published literature, focus groups and interviews with key informants, document review and a consultation workshop. The Wagner model was used as a benchmark to assess the kinds of changes that were the centre of interest to this study. The report then, is about internal processes in those three CHS organisations that were developed through the establishment of their EliCD programs as well as the broader system change that was needed to develop integrated and multidisciplinary care. The project found that change necessarily occurred at three levels: · The practitioner, · The organisation, and · The broader service system. All levels of change are essential for effectiveness and sustainability of the EliCD.

Problem Gambling Resource Kit
The Department of Justice has produced the Problem Gambling Resource Kit – A guide to assist Health and Welfare workers who encounter problem gambling issues. The kit has been developed in conjunction with Gamblers Help services, which are the primary source of problem gambling advice and support. Gambler’s Help services offer assistance to problem gamblers, people at risk of problem gambling, their friends and families through a Victoria-wide service network. Importantly, they also provide a range of services to health and welfare workers to support them in managing clients who may have gambling problems, and to raise awareness of problem gambling issues. Gamblers Help Northern will take carriage of the dissemination of the kits to relevant health and welfare workers in the NMR. This is the first comprehensive problem gambling resource kit of its kind, with a well designed format for quick access and navigation. In particular, Section 4: Tools for workers: identifying and responding to problem gambling contains some of the most important guidance and tools for workers in responding to clients’ gambling issues including: *What can I do as a professional? * How can I tell if someone has a gambling problem * What are the signs I should look out for * How do I approach the issue without offending my client * What GH Services can do? In most cases, often clients facing problem gambling issues will present to the service system with a range of other issues including mental health, relationship/family, drug & alcohol, housing, legal and emergency relief. Gamblers Help Northern recognises the benefit in providing a seamless access to clients experiencing multiple issues, other issues treated parallel to the gambling problem will maximise an effective service approach and essentially a better outcome for clients. In our strategic planning exercise, community health centres were identified as a primary avenue to reach specific health and welfare workers within the health promotion, allied health, medical, EICD, counselling and service co-ordination teams. It is intended that the Kit be used to help identify people presenting at a wide range of health and welfare settings with gambling-related issues; whether these issues are related to their own gambling or the gambling of family members. It aims to assist in the early identification of gambling problems as well as in the identification of people with longer standing problems, and will support health and welfare workers in making effective referrals and other interventions on behalf of their clients, in collaboration with Gamblers Help services. Well for Life Resource Book
The Well for Life initiative aimed to improve the nutrition and physical activity for older people at home. BNPCA was invoved during the Phase 2 of the project and involved Planned Activity Group Staff, Volunteers and Carers as participants in our project. Training was provided by a dietitian and physiotherapist, and armed the participanst with additional knowledge to support integration of the concepts within their care-giving environment. A Resource Book has been developed as part of the follow up to the Well For Life Initiative that took place in the BNPCA catchment during 06/07. It is hoped the resource book will consolidate the learning achieved during the project phase and promote the ongoing implementation of the Well for Life principles and practices in Planned Activity Groups.

Workshop 1 - Focus on Equity
The first workshop in our series of "Focus on Equity" Workshops was held on Tuesday 4th March 2008. Use the links in this report to access the presentations on the day. Anita Thomas provided a wonderful demonstration of all the influences on our health and well being, other that direct health (medical) factors. Helen Clough and Ben Pollard gave us a brief overview of the demographics in our local government areas of Banyule and Nillumbik and this information would available on their respective LGA websites. Mark Boyd then went on and demonstrated the unfair or avoidable differences in health outcomes experienced by members of our communities. Mark then distributed the draft Equity Tool and gave us a brief overview of the triangle which encourages to go beyond equity of access to consider ensuring equity of opportunity and measuring outcomes.
