As of 1 July 2010, three new Primary Care Partnership boundary configurations will commence operations in the Department of Health, North and West Metropolitan Region. The Victorian Minister for Health endorsed the changes in December 2009 based on the following rationale: * Previous configurations had proved challenging for some PCP agencies. * Population growth projections indicate a significant increase for the N&WMR. There is a need to address growth and support appropriate service developments and responses. * A need for robust area-based planning to enhance planning and service delivery. PCP member agencies and staff have worked closely with the region over the past six months to facilitate the transition to new entities which are now ready to commence operations. Attached is an information sheet developed by the region to inform you on transition progress, processes and contact information regarding the new PCPs.
