Integrated approach to Chronic Disease Programs Launch

A successful launch of our streamlined referral framework was held on Thursday 2nd July 2009. Service providers in Banyule, Nillumbik and Darebin have been working together to streamline referral and access to programs for people with chronic conditions so that: If you have a chronic condition you get the right help at the right place at the right time. If you want to make a referral for a chronic condition you can make one referral to get the right help at the right place at the right time. The time and effort involved in maintaining information, contact details and eligibility criteria for each program and organisation across the catchment is eliminated.
