Practice

The Practice section contains four modules:

Module 3.1 Initial Contact

Module 3.2 Initial Needs Identification

Module 3.3 Assessment

Module 3.4 Care Planning

Each one has been designed as a separate learning module. It is not necessary to complete all modules of this Practice Section. Learners should only complete those modules which are relevant to their job role. For example:

  • A new Receptionist may choose to complete Module 3.1 Initial Contact and Module 3.2 Initial Needs Identification only.
  • New Service Coordination Workers (Intake Workers) may consider completing all the modules of this Section.
  • A new practitioner with previous experience in another agency or program may need to complete Module 3.4. Care Planning only.
  • Workers may need to be aware of all parts of Service Coordination, in such cases we recommend completing all modules in this section.

If you are uncertain about which parts to complete consult your Team Leader or Manager.
