Service Coordination: What? Why? How? is comprised of five learning modules. Learners may complete some or all of the modules. There are a variety of learning pathways through the Service Coordination SPTM (see diagram below) with agencies able to tailor learning pathways to suit individual needs and complement orientation or induction programs.

Learning Pathways through the five modules Module 1: Why do we need Service Coordination? Module 2: What are the benefits? Module 3.1 Initial Contact Module 3.2: Initial Needs Identification Module 3.3: Assessment Module 3.4: Care Planning Module 4: How and when do I make a referral? Module 5: What resources and supports are available?

For example a new Receptionist may only need to complete the introductory Modules 1 and 2, plus the part on Module 3 on Initial Contact, whereas a new Service Coordinator or Duty Worker/Intake Worker may need to complete all modules.
