activities. It has been developed to complement agency Orientation and
Induction Programs.

to respond to consumers’ and carers’ needs.
- An understanding
of the benefits of Service Coordination.
- An overview of the elements of
Service Coordination including Initial Contact, Initial Needs Identification,
Assessment, and Care Planning.
- An overview of how Referral fits within
Service Coordination.
- An understanding of the resources available to
support Service Coordination including the Service Coordination
Tool Templates and Guidelines.
- A general understanding of Practices, Processes,
Protocols and Systems (PPPS) and their role in guiding agency practice.
