This is a self paced learning module for staff involved in Service Coordination activities. It has been developed to complement agency Orientation and Induction Programs.

  • An overview of the elements of Service Coordination including Initial Contact, Initial Needs Identification, Assessment, and Care Planning.
  • An overview of how Referral fits within Service Coordination.
  • An understanding of the resources available to support Service Coordination including the Service Coordination Tool Templates and Guidelines.
  • A general understanding of Practices, Processes, Protocols and Systems (PPPS) and their role in guiding agency practice.
