Term Definition

Access

The right of, opportunity and means for entry.

Assessment

A decision-making methodology that collects, weighs and interprets relevant information about the client. Assessment is not an end in itself but part of a process of delivering care and treatment. It is an investigative process using professional and interpersonal skills to uncover all issues and to develop a care plan.

BATS

The policy and operational framework that will assist Primary Care Partnerships (their organisations and the organisations’ practitioners) to achieve functional integration. This functional integration will be facilitated through a systemic and consistent approach to assessment and service delivery.

BATS strategy

The agreed proposal for the ongoing planning, implementation and management of the Better Access to Services Operational Framework as part of each Primary Care Partnership’s Service Coordination model. The Better Access to Services strategy is part of the Primary Care Partnership Community Health Plan, and is based on shared agreement about the Service Coordination model. The purpose of this strategy is to improve people’s access to services by making them more visible and hence improving health and well being outcomes.

Best Practice

A concept of organisational change and improvement that has been adopted from the industrial sector where it is seen as the pursuit of ‘world class’ performance. Best practice is considered to be a comprehensive integrated and cooperative approach to the continuous improvement of all facets of an organisation’s operations.
