The Department of Human Services has developed a number of resources to support Service Coordination around privacy and consent.

What’s the purpose of the Consumer Consent Form and Your Information – it’s Private brochure?

To meet privacy consent requirements many agencies use the Consumer Consent Form and the Your Information – it’s Private Brochure. The Consumer Consent Form and brochure provides a uniform approach to obtaining consumer consent for the sharing of consumer information and meets requirements of the Victorian Health Records Act 2001. A copy of the brochure is set out below.
