The Service Coordination Model and PPPS developed by your PCP will outline how Referral will happen in your local area.

Typical Referral PPPS include:

  • Use of the Service Coordination Tool Templates including quality standards for information collection and documentation.
  • Business Rules covering use of fax, email or e-referral systems.
  • Communication and feedback requirements.
  • Consent and privacy.
  • Response times for:
    • referrals made for clients in crisis or with urgent needs, and
    • feedback.
