Consumer Consent TemplateThe Consumer Consent Template

To meet privacy consent requirements many agencies use the Consumer Consent Template and the Your Information – it’s Private Brochure. The Consumer Consent Template and brochure provides a uniform approach to obtaining consumer consent for the sharing of consumer information and meets the requirements of the Victorian Health Records Act 2001. A copy of the Consumer Consent Template and brochure is set out below and can be downloaded from http://www.health.vic.gov.au/pcps/publications/index.htm

Please note, your agency may have its own consent forms and/or brochure. Discuss this with your Supervisor or Manager.

The Consumer Consent Template is used to obtain consent from the consumer before any disclosure of information for any secondary purpose (such as a referral) either inside or outside the agency. This list checks that the consumer’s consent is:

  • Informed - what and why.
  • Freely given - unconditional right of refusal.
  • Specific - as documented.
  • Current - will be reviewed and kept up-to-date.

Find a copy of the Consumer Consent Template used by your agency, make sure you know how to fill it in correctly and are familiar with filing procedures.

Please note, your organisation may also require a separate consumer consent form for service provision, treatment, payment of fees, transport etc.
