All staff involved in Service Coordination and/or service delivery will make referrals at some time. Therefore it is important that all staff are familiar with statewide requirements, local PCP PPPS and agency practice which govern how referrals between agencies will occur.

What is expected of staff involved in Referral?

There are four key requirements for all staff making inter-agency referrals. Staff must:

  • Photo of man receiving dental procedureMake referrals in accordance with organisational guidelines, policies, procedures and work instructions.
  • Use the Service Coordination Tool Templates to share consumer information when making referrals between agencies. Some agencies also use the Service Coordination Tool Templates for making internal referrals.
  • Meet privacy requirements including providing consumers with a copy of the brochure Your Information – it’s Private or equivalent agency information.
  • Adhere to local PCP Referral PPPS including using agreed fax cover sheets, e-referral systems, feedback forms etc.

Complete the following activity:

Obtain a copy of, and familiarise yourself with, your agency’s Referral policy, procedures and work instructions. If your agency is a signatory to a PCP Referral Protocol or PPPS, obtain a copy and familiarise yourself with the content and practice requirements. If your agency or program is not a signatory to Referral PPPS, ask your manager why.
