Referral may occur at anytime, out of any or all the elements of Service Coordination.

A consumer may be referred for any of the following:

  • Information e.g. services available, health promotion, chronic disease management.
  • Health Promotion activities, including groups, information sessions or skills development.
  • Initial Needs Identification.
  • Assessment or further Assessment including Service Specific, Specialist or Comprehensive Assessment.
  • Service provision.
  • The development of a Service Coordination Plan.
  • Reassessment, monitoring and review.
