2. How does Referral fit into Service Coordination?

Referral may occur at anytime, out of any or all the elements of Service
Coordination.

A consumer may be referred for any of the following:
- Information e.g. services available, health promotion, chronic disease
management.
- Health Promotion activities, including groups, information sessions
or skills development.
- Initial Needs Identification.
- Assessment or further Assessment including Service Specific, Specialist
or Comprehensive Assessment.
- Service provision.
- The development of a Service Coordination Plan.
- Reassessment, monitoring and review.
