1. What is Referral?

Referral describes the transmission of personal and/or health information
relating to an individual from one service provider to another service provider(s)
with the individual’s consent and for the purpose of further assessment,
care or treatment.
Referrals are made from all elements of Service Coordination. For example:
- A staff member involved in Initial Contact may refer a consumer on
to another, or more appropriate, service.
- An Initial Needs Identification practitioner
may identify referrals as part of their screening role.
- Practitioners may
identify further assessments and treatments required to meet a client’s
needs as part of the assessment process.
- Referrals often occur out of Care
Planning which aims to coordinate a package of care and services for
a consumer.
As the needs of clients change, further referrals may be required.
Service Coordination has been great! In the past we never knew what happened
to the referrals we sent off. Sometimes we wondered if they had been received
or acted on. Now we not only get automatic feedback that our referral has been
received and acted on but we also get feedback on the outcomes later on.
