Service Coordination Tool Templates (SCTT) 2009 in Word and PDF .

The 2009 Service Coordination Tool Templates (SCTT) 2009 are now available for download in both Word and PDF format from the DHS website. The SCTT 2009,a suite of templates, was released for use as of the 1st of July 2009. The SCTT was developed to facilitate and support service coordination practice within Victoria. They can; *support service providers to collect and share common consumer information *assist service providers to record consumer consent to share infromation assist service providers to make quality referrals, provide feedback and devlop care plans

GP Feedback Project

The BNPCA has now completed the project on "Improving Feedback to GPs". The project outcomes are: * GP Feedback Project Report * Good Practice Guidelines for GP Feedback * Templates for feedback about Assessment Outcome & Discharge Summary BNPCA acknowledges the contribution to this project by Simon Van Dam and Darren Johnstone who at the time were Occupational Students at Monash University. Agencies have committed to using the Good Practice Guidelines for GP Feedback as a quality standard to assess their own procedures and practice by. If needs be they will adapt their procedues to comply with these guidelines.

Mental Health Services in Banyule and Nillumbik

This brief contact list was put together to provide a listing of Mental Health agencies within Banyule and Nillumbik. The information contained in this listing is taken principally from the DHS Mental Health Services web-site and from agency pamplets/ web-sites. The Banyule Nillumbik Primary Care Alliance (BNPCA) and the North East Valley Division of General Practice (NEVDGP) are currently running a joint project to produce a comprehensive Mental Health Services and Support Directory for Banyule and Nillumbik. This Directory will have detailed information around service description, eligibility and access criteria, contact hours and other relevant details. This Directory should be available in July 2009 on both the NEVDGP and BNPCA web-sites. Please contact Sally Berger for further information.

ELECTRONIC SERVICE COORDINATION SYSTEM EVALUATION PROJECT

The Management Committee of the Electronic Service Coordination System (E-SCS) has commissioned Ocean Informatics to perform this evaluation of the current state of the application and its use in the Eastern region. E-SCS has been implemented in an increasing number of sites over the past few years but it has only been in the last 12 months that a concerted effort has been made to train staff and encourage wider use. This recent effort has resulted in a very encouraging increase in the e-Referral traffic amongst a wide range of users. This report is designed to understand how the current system is being used and accepted and what is needed to sustain and increase its usage. There is also a need to understand how the system fits with other initiatives in the electronic Referral domain in Victoria and further afield. The evaluation process undertaken used two complementary approaches. It commenced with an online survey to get feedback about the broad on-the-ground experience of E-SCS. Registered users and managers were emailed invitations to participate in the survey; results were collated and analysed. Results from the survey were shared, and the issues raised in the survey were discussed, with focus groups and stakeholders to obtain more detail and practical insight. Conclusions and recommendations comprise the final part of the report, based on collation and studying the combined feedback from the online survey, focus groups and stakeholders.

Improving Service Acess for Refugee Clients

The BNPCA and NCMPCP recently ran a local celebration in recognition of the service coordination work carried out by organisations and agency workers across the catchments and the achievement of the Victorian Service Coordination Practice Manual. The forum highlighted the issues for refugee clients accessing services and some local work that has improve this situation. The presentations from the forum are included for your information.

HACC Assessment Framework

The HACC Assessment Framework sets out progran policy for Assessment as a HACC funded activity. It describes in detail the requirements for the delivery of a Living at Home Assessment which includes a home-based holistic assessment of need and service-specific assessments as key components. In most instances theses Assessments will occur wherever the client is currently living in the community. The Framework also describes related processes such as Client Care Coordination and Supported Acess. Both of these processes are critical adjuncts to assessment for specific client groups. The policy described in the Framework builds on existing assemssment expertise and good practice within the HACC Program.. To this end, the Framework will be an important reference point for promoting more consistent assessment processes and practices across Victoria.

Service Access Models: A Way Forward

Integrated service access models support efficient, effective and consistent consumer access to services. They also reflect individual organisation characteristics and local circumstances, and further embed community health within the service system. This resource guide has been developed to assist Community Health Service (CHS) managers to review, select, implement and evaluate service access models. It outlines the importance of service access models in supporting Service Coordination and provides a summary of the context. It identifies various service access models and describes and compares benefits and challenges associated with each. Factors influencing the selection of a service access model and the desirable outcomes an effective service access system can support are articulated. It includes implementation and change management strategies, as well as evaluation methodologies.
